Johnson PS: Transition Info
At JPS, we understand that frequent moves and transitions are a part of military life. Below you will find resources and information that we hope will assist with any future transitions. Reminder that you can only attend a base school if you live on base AND the sponsor is active duty.
- Updated Immunization record with a flu shot that has happened within the calendar year
- Birth Certificate - original or certified copy (or non-expired passport)
- Original Orders for Camp Lejeune
- Copy of the lease AGREEMENT, first page only (different from acknowledgement
- Any IEP/504/Confidential medical records that pertain to the child attending school must be brought in to be copied and kept on file
- For Pre-K enrollment: Child must turn 4 ON OR BEFORE September 1st.
- If active-duty sponsor is a step-parent to the child, please provide a copy of the marriage certificate and the military ID of the spouse
- A withdrawal form needs to be filled out at least two weeks (but no more than a month) prior to disenrollment. The Registrar, Ms. Monica Hernandez, needs to be notified of the disenrollment
- Copy of the sponsor’s PCS or EAS orders
- Clear all debts: Return library books and settle cafeteria accounts
- ALL OCONUS PCS should request physical records from the school. If PCS is within the United States, the records request should be made by the next school the child is attending.
- IMPORTANT NOTE: JPS records are federally protected. A release from the new school will need to be signed by the parent/guardian once the child is being enrolled in the new school.
