School Registration Packet for New Students

The sponsor must fill out a packet for each student:  Kindergarten - 5th grade Registration Packet

Sure Start Registration Packet

 

Upon arrival at Yokota AB, Sponsor must present:

  • Copy of military sponsor's orders or Letter of Employment, SF50 (provided by unit), and a copy of ID card for all local hire DOD civilian.
  • Proof of Birth (copy of the birth certificate or passport is acceptable)
  • Student's immunization record.
  • Command Sponsored letter (if applicable)
  • Area clearance message or dependent entry approval message listing school age dependents by name. (Navy or Marines)

 

Returning Student Registration Information

Spring Registration will begin on March 2nd and will take place at Mendel Registrar's office, this year.  Parents are asked to stop by the school, at the Registrar's office, to sign the required forms.

Sponsors should complete the registration process as indicated for each option as it applies to their child(ren):

1. RETURNING STUDENTS: MARCH 2 - 30 from 8:00-11:30 & 12:30-15:00 For ALL current students in Sure Start, Kindergarten - 4th grade.

2. NEW KINDERGARTEN STUDENTS: APRIL 9 - 27 from 8:00-11:30 & 12:30-15:00.

3. CURRENT 5TH GRADE STUDENTS who are transferring to Yokota Middle School: Ms. Maria Clemons, Registrar of YMS, will send the registration packet to the parents of the current 5th graders. Be on the lookout!

4. CURRENT STUDENTS WHO ARE MOVING: 
Submit a withdraw request form 5 DAYS prior to their last day of school.

5. CURRENT STUDENTS WHO ARE UNDECIDED: 
Re-register tentatively by following option 1 or 2 in a timely manner.

6. Sure Start applications will begin to be accepted on April 9th, at the same time as the New Kindergarten Student registration.
